When we recognize concerns early, we intervene quickly, address issues proactively, and help prevent hospitalizations or serious complications. Our team relies on consistent observation, clear communication, and the confidence to act.
I encourage staff to speak up whenever they notice even small changes in a resident. During orientation and Town Hall meetings, we reinforce the use of the Stop and Watch tool. Staff know they must report any change in condition to a Nurse Manager or to me right away. We then assess potential causes, such as a urinary tract infection, and determine next steps.

When changes continue, we update the CBA and service plan to reflect evolving needs. Ongoing education helps staff understand why documentation and timely updates matter. By setting clear expectations and responding quickly, we build a culture where speaking up protects residents.
Daily routines support early detection across the community. Stand-up meetings and rounding create structured opportunities to share observations. Care Partners complete walking rounds during shift changes and document behavioral or physical changes. During shift report, they communicate concerns directly to the incoming team.
This process allows us to act quickly. We schedule evaluations with our Nurse Practitioner or coordinate with families to arrange follow-up with the primary care provider when needed. Because we communicate consistently, we reduce delays and close gaps in care.
Early identification builds family trust. When we adjust care plans promptly and communicate clearly, families feel confident in our approach. They see that we remain attentive and responsive as needs change.
